Indiana University Bloomington Navigator Program
Indiana University Bloomington senior nursing and second year medical students work with the Indiana University Health Bloomington Transitional Care Nurse manager to conduct home visits to patients discharged from acute care who are high risk for readmission.
Rush University: Implementing the Ambulatory Integration of the Medical and Social (AIMS) Model
This project provides Rush University Medical Center interprofessional providers, residents, and students an opportunity to work collaboratively to best meet the needs of their patients. At partner clinics, any member on the care team can place referrals to the Rush Health and Aging Department (RHA) for social work follow-up if they identify their patient (18+, public or private payer) has a psychosocial need or is facing barriers to following their health care plan. The social worker provides wrap-around supports that engage various interprofessional healthcare and service providers in ensuring a patient’s range of biopsychosocial needs are addressed. This collaboration will reinforce the paradigm shift within primary care of treating a patient holistically, and the inclusion of social work students and medical residents in this project is significant because it exposes them to important practices and competencies in integrating primary care with care management and psychosocial supports.

ETSU Joins Nexus Innovation Network
MINNEAPOLIS – East Tennessee State University has joined the National Center for Interprofessional Practice and Education's Nexus Innovation Network.
As a member of the Nexus Innovation Network, ETSU is now one of 71 sites in 35 states to test new models and methods of integrating health care practice and education. Each of the 97 projects in the Nexus Innovation Network focuses on the real-world testing of interprofessional strategies with the potential to improve the experience, outcomes and costs of health care.
Train-the-Trainer Faculty Development Program: University of Missouri, September 11-14, 2017
This three-and-a-half day curriculum was co-developed by experts from three universities with established IPE training programs and a national advisory committee. Training sites will provide content, skills, and strategies for facilitating and coaching IPE teams. The purpose of this program is to prepare health professions faculty and collaborative practice clinicians from all professions to lead IPE efforts and promote interprofessional team-based care.

Introducing Collaborative Care: Teaching Basics of Interprofessional Education in an Online Environment
This chapter describes the development and implementation of an innovative course in interprofessional education (IPE), which ensures a large number and variety of health professions students have the appropriate foundations to collaborate. A description of the institution and the process of implementing interprofessional education is followed by a presentation of challenges and then solutions to address them in the creation of the course. Future research avenues in interprofessional education will be explored.
