Engaging Older Adults and Family Caregivers
Community Catalyst’s Center for Consumer Engagement in Health Innovation partnered with Rush University Medical Center’s Geriatric Workforce Enhancement Program (GWEP), CATCH-ON (Collaborative Action Team training for Community Health – Older adult Network), to develop a suite of consumer-facing, age-friendly “4M” materials. The 4Ms are a framework first developed by the Institute for Health Improvement and The John A. Hartford Foundation as part of their Age-Friendly Health Systems initiative - an evidence-based approach to high quality care for older adults.
GWEP-CC Age-Friendly Health Systems Case Studies
This series of Case Studies is focused on primary care experiences implementing the 4Ms (Mentation, Medications, Mobility, What Matters) of Age-Friendly Health Systems (AFHS) in their practices. Case study authors participated in the Geriatrics Workforce Enhancement Coordinating Center (GWEP-CC) AFHS Action Community (2020-2021). The Institute for Health Care Improvement (IHI) has recognized these practices as an Age-Friendly Health System Participant (Level-1) or Age-Friendly Health System – Committed to Care Excellence (Level-2).
Community Grand Rounds: An Inconvenient Care
Grand rounds are an integral component of medical education. It helps doctors and other healthcare professionals keep up to date in important evolving areas which may be outside of their core practice. In the hospital or health system, they are hosted in an auditorium or symposium for everyone to gather and continue their learning. "Medicaid ACO Checklist for Advocates "
The Center for Consumer Engagement in Health Innovation has created a toolkit to guide advocates as they assess and work with state’s Medicaid ACOs (Accountable Care Organizations). Each section contains a list of items to consider as Medicaid ACOs are designed and implemented. Creating this type of system takes time and is an evolving process. The most fundamental part of each component is to ensure that the consumer voice is present. Consumer advocates can play a role in the design, implementation and oversight of Medicaid ACOs.
