Professional Gaps and Educational Needs: Despite high functioning interprofessional teams being present at acute care and inpatient rehabilitation, a significant amount of information is lost in the transition between facilities. A retrospective chart review revealed 30-80% of information related to common complications after stroke was missing.

Objectives:

The Centers for Disease Control and Prevention (CDC) will host two webinars this month to discuss an education program on social determinants of health.

The webinars will focus on the Health and Well Being for All program, which originated as materials developed for CDC by the Public Health Foundation. The program’s “Meeting-in-a-Box” system is designed as an innovative and interactive way to educate health care professionals about the social factors that affect overall health.
