The Geriatric Transitions Objective Structured Video Examination (GT-OSVE) is a series of 3 video cases depicting the same hypothetical patient undergoing transitions of care in different locations: hospital to home, skilled nursing facility to home, and home to an assisted living facility. The GT-OSVE may be used as an evaluative tool to assess trainees' ability formulate transitions plans and to assess team functioning. Alternatively, the GT-OSVE may be used as an educational tool for teaching about transitions of care.

IPAS: Interprofessional Attitudes Scale

IPAS is a scale designed to assess attitudes that relate to the 2011 Core Competencies for Interprofessional Collaborative Practice. IPAS is one of the first scales to focus specifically on the Core Competencies. IPAS consists of 27 items in 5 sub-scales, which we have called "Teamwork, Roles, and Responsibilities", "Patient-Centeredness", "Interprofessional Biases", "Diversity & Ethics", and "Community-Centeredness". IPAS was created from factor analysis of survey data collected from over 700 student respondents at the University of Utah Health Sciences Center in 2012.

The Interprofessional Collaborative Competency Attainment Survey is a 20 item self-assessment tool. Its items cover aspects of trainee roles on a team and use of interprofessional practice team approaches to patient care. 

The ICCAS is intended to measure the self-reported competencies of interprofessional care in interprofessional education programs (Archibald, Trumpower, & MacDonald, 2014)

The SF-36 is a multi-purpose, short-form health survey with only 36 questions. It yields an 8-scale profile of functional health and well-being scores as well as psychometrically-based physical and mental health summary measures and a preference-based health utility index. It is a generic measure, as opposed to one that targets a specific age, disease, or treatment group.

HEDIS is a tool used by more than 90 percent of America's health plans to measure performance on important dimensions of care and service. Because so many plans collect HEDIS data, and because the measures are so specifically defined, HEDIS makes it possible to compare the performance of health plans. Health plans also use HEDIS results themselves to see where they need to focus their improvement efforts.
