The Rapid Geriatric Assessment (RGA) is a brief screening tool that:
• provides a brief, reliable method for detecting common geriatric syndromes, including frailty, sarcopenia, geriatric anorexia, and cognitive function; 
• helps identify problems for which early interventions are most likely to be successful; and 
• enables tracking of changes over time (annual screening checks)

GT-OSVE: Geriatric Transitions Objective Structured Video Examination

The Geriatric Transitions Objective Structured Video Examination (GT-OSVE) is a series of 3 video cases depicting the same hypothetical patient undergoing transitions of care in different locations: hospital to home, skilled nursing facility to home, and home to an assisted living facility. The GT-OSVE may be used as an evaluative tool to assess trainees' ability formulate transitions plans and to assess team functioning. Alternatively, the GT-OSVE may be used as an educational tool for teaching about transitions of care.

PROMIS® stands for Patient Reported Outcomes Measurement Information System, which is a system of highly reliable, precise measures of patient–reported health status for physical, mental, and social well–being. PROMIS® tools measure what patients are able to do and how they feel by asking questions. PROMIS measures can be used as primary or secondary endpoints in clinical studies of the effectiveness of treatment, and PROMIS® tools can be used across a wide variety of chronic diseases and conditions and in the general population.

The Safety Organizing Scale was developed by Vogus and Sutcliffe at Indiana University School of Nursing and IU school of Medicine. This is a tool that is robust , valid and reliable . We will be using the measuement tool with the NEPQR grant to cultivate IPCP enviroments both in Acute ACC and rural health clinic sites .

The Interprofessional Education Assessment and Planning Instrument for Academic Institutions was designed to do the following: 1) assist institutions in gauging where they are in development of Interprofessional Prevention Education and 2) assist institutions wishing to further advance Interprofessional Education with an emphasis on prevention.

This tool will allow institutions to asses where they are in the development of IPE on their campuses.

The TeamSTEPPS Teamwork Perceptions Questionnaire (T-TPQ) was developed to assess health professionals' perceptions of interprofessional teamwork within an organization and group-level team skills and behavior.

T-TAQ is used to measure how individuals approach team related issues. The TeamSTEPPS Teamwork Attitudes Questionnaire (T-TAQ) was developed to assess the impact of interprofessional education on health professionals' attitudes, knowledge, and team skills.
