This report from the Milken Institute Alliance to Improve Dementia Care centers around two overarching themes: (1) strengthening the infrastructure among health care, long-term, and community-based organizations to achieve greater health equity for people living with dementia and their caregivers and (2) expanding dementia-friendly networks and workplaces in racially and ethnically diverse communities. This report presents actionable recommendations to build health equity by reducing disparities in dementia prevention, detection, diagnosis, and care.

The report identifies three care models that states can choose to achieve full integrative care: (1) improved Fully Integrated Dually Eligible Special Needs Plans (FIDE-SNPs); (2) Programs of All-Inclusive Care for the Elderly (PACE); and (3) a flexible model building off the managed fee-for-service model implemented in Washington State.

This report summarizes the presentations and discussions from the Inclusion Across the Lifespan Workshop (IAL-II) including the topics of inclusion and exclusion criteria; study designs and metrics; recruitment, enrollment, and retention; and data analysis and study interpretation.

This Technical Brief applies a framework to create a map of the currently available evidence on patient and family engagement strategies that have been used to help people manage chronic conditions. The report focused on research regarding from national organizations focusing on health system and community/policy levels.
