This toolkit, developed by the Rush Alzheimer’s Disease Center, is designed for dementia support group facilitators who want to learn and grow in their practice. The Toolkit Project was developed based on insights gained from a dementia support group for adults with younger-onset Alzheimer’s disease and their caregivers. The toolkit includes information about support group facilitator skills, common themes expressed by people living with dementia and their family members, topics for encouraging discussions, and resources to share with support group members.

This toolkit webpage offered by the Center for Aging Research and Education (CARE) at the UW-Madison School of Nursing includes materials and videos to build communication and advocacy skills to respectfully engage people living with dementia. Students, health care or other professional staff, family caregivers, local groups, or local businesses can use the materials or videos to learn more about dementia or share information with others in their workplace or community. Topics include role playing, communication tips, family caregiver tips, and more.

The Oral Health Nursing Education and Practice (OHNEP) program developed the COVID-19: Oral Health Resource Kit for nursing and health professions educators to incorporate valuable interprofessional teaching resources in classroom and clinical simulation settings to demonstrate the connections between oral health and COVID-19. These resources include an unfolding case study, patient health literacy, a slide deck and a comprehensive COVID-19 and oral health resource list.

This reference guide summarizes a set of curated resources to help key stakeholders — including health plans, providers, policymakers, and community-based organizations — better understand both the needs of the dually eligible population and emerging evidence and practical approaches for advancing Medicare-Medicaid integration.

The Gerontological Society of America and its Academy for Gerontology in Higher Education designed this toolkit, with support from AARP, to provide resources to advance age inclusivity in institutions of higher education. These tools and resources may be adapted to meet your institution's approach to making the case, building relationships, addressing ageism, crafting new efforts, and conducting assessments. The suite of tools can be used by faculty, students, administrators, and other campus leaders.

This toolkit from the Gerontological Society of America is intended as a supportive document of practical approaches, educational resources, and validated clinical tools to help primary care teams implement the KAER Model in their own initiatives related to brain health and timely detection of cognitive impairment. Information on the different types of dementia is also included. Medical educators and medical students may also find the toolkit's content useful.

The AGS Cognitive Screening Toolkit includes free open access cognitive screening tests that are recommended by the USPSTF. The toolkit can serve as a resource for clinicians to better understand cognitive screening tools, which have been validated in Primary Care settings and are available for free. This toolkit can help in selecting the appropriate cognitive screening tool to use with primary care patients.

This toolkit by the Eldercare Workforce Alliance was published to help those interested in starting a statewide coalition focused on the workforce that cares for the aging population. The toolkit includes: The Issues, Retooling Recommendations and Strategy, and State Coalition Toolkit. These include step by step instructions to carry out this formation of a statewide coalition with examples from the North Carolina Eldercare Workforce Coalition.

The comprehensive geriatric assessment (CGA) toolkit for general practitioners and medical and health professionals working in primary care settings explains what comprehensive geriatric assessment is, in what circumstances to use it, and how it is performed with planning and involvement of social services. The toolikit includes guides on specific medical issues covering a range of conditions and situations, from bone health to end of life care. All content can be downloaded in PDF form.
