This toolkit provides different resources and information for clinicians on ACE screening and clinical response. Resources are organized by type, topic, and by stage. Resources can also be found by using the advanced search. ACEs Aware is sharing learnings, research, practices, and experiences in hopes to unite all to advance the standard of care for ACEs and toxic stress.

This resource provides practice tools and resources to help those who work with patients and clients with memory concerns and to support their care partners. Included in this toolbox are a protocol practice tool for cognitive impairment, a decision support tool for dementia care, a protocol practice tool for mid- to late-stage dementia, care coordination practice tools, and tips and action steps to share with a person diagnosed with Alzheimer's.

The National Resource Center on LGBTQ+ Aging, a program of SAGE, is the country's first and only technical assistance resource center focused on improving the quality of services and supports offered to lesbian, gay, bisexual, and/or transgender older adults, families and caregivers. Established in 2010 through a federal grant from the U.S. Department of Health and Human Services, the National Resource Center on LGBTQ+ Aging provides training, technical assistance and educational resources to aging providers, LGBTQ+ organizations and LGBTQ+ older adults.

Alzheimer’s disease and other forms of dementia have physical and psychological effects that can make it difficult for seniors to maintain their independence. It’s important to understand the changes that can occur with one of these diseases. Having dementia puts older adults at risk of several types of crises, including wandering, increased aggression, hallucinations and depression. This guide covers the most common crisis situations and offers tips for preventing them.

The Dementia Friendly @ Work training is a one hour education session for staff of business and service organizations as well as other community settings. Anyone can deliver the training, using the Trainer's Guide to prepare and deliver the content. The training focuses on understanding dementia, communication tips and taking action to create an environment that is safe, respectful, and welcoming for people living with dementia. More than 100,000 Minnesotans have dementia or a related disorder. It’s projected that the number of Alzheimer’s cases and other dementias will triple by 2050.

This toolkit authored by Edward Ratner contains resources for teaching learners how to obtain information from older patients and their family or other caregivers that aligns with their specific health outcome goals and care preference. These resources provide content related to the "What Matters" concept in the 4Ms framework of an Age-Friendly Health System. It contains introductory material appropriate for undergraduate and graduate level learners. This toolkit contains two components: a Learner Module and a compendium of Teaching and Learning Resources.

This toolkit was authored by Brian J. Isetts in collaboration with Kristine Talley and supported by colleagues of the Interprofessional Geriatrics Coordinating Council. It contains resources for teaching older persons and family caregivers how to conduct a medication self-management assessment. It also contains introductory material appropriate for undegraduate and graduate level learners, and is intended to help older persons confidently manage their medications. The toolkit has two components: a learner module and compendium of teaching and learning resources.

MedEDPORTAL is a MEDLINE-indexed, open access journal of teaching and learning resources in the health professions. The publications on this site are stand-alone, complete teaching or learning modules that have been implemented and evaluated with medical or dental trainees or practitioners. Each submission is reviewed by editorial staff and external peer reviewers. This research instruments would be particularly helpful for physicians and nurse practitioners.
