These best practice tools incorporate the expertise of clinical and community-based service providers. Since the initial creation of the Healthy Brain Initiative (HBI) in 2005, HBI partners have worked together to implement public health strategies that promote brain health, address dementia and support people with dementia and their caregivers. The Healthy Brain Initiative Road Map Series guides this effort by creating a framework for public health action.

MyHealthPriorities: Identifying My Health Priorities is a tool for patients to prioritize what is important to them, which can be shared with health care teams. MyHealthPriorities was created to help people (together with their caregivers if desired) discover what matters most to them and to help identify their Health Priorities in order to make better care decisions with their health care team. Requires either creation of free account or can sign in as "guest".

It also helps us create the foundation of a care plan that’s right for us — a plan that will be available when the need arises. The Conversation Project wants to help everyone talk about their wishes for care through the end of life, so those wishes can be understood and respected.

People with communication disabilities often find it hard to get the information and communication supports they need to fully access health services and programs. This can lead to less-than-ideal outcomes for everyone involved. As a health professional, learn how you can support effective communication access to promote equitable care and better health. ACCESS is a framework of six core principles to help lessen communication barriers. Always check your patient's communication needs before starting care using the ACCESS tool.

The Public Health Center of Excellence on Dementia Caregiving (PHCOE-DC) is a collaboration between the CDC and the University of Minnesota to support state, tribal and local public health agencies nationwide in implementing actions that protect the wellbeing and meet the needs of family members, friends, partners, and other unpaid caregivers of people with dementia, in line with CDC’s Healthy Brain Initiative State and Local Public Health Partnership to Address Dementia: The 2018-2023 Road Map.
