This practice tool from ACT on Alzheimer's provides clinicians a streamlined protocol for managing cognitive impairment and guiding decisions for screening, diagnosis, and disease management. This tool incorporates current best practices for efficient and appropriate dementia care, and includes links to resources. This tool can be used nationally.

NCHWA’s Nursing Workforce Dashboard visualizes data from the 2018 National Sample Survey of Registered Nurses (NSSRN), which includes detailed information on the nursing workforce in the United States. Use this tool to: view data on the nursing workforce landscape, including demographics, employment, and education for RNs, NPs, and APRNs; analyze results for specific subsets of the data using filters by location, license type, and employment status; and access over 100,000 unique data points from the NSSRN.

The goal of this Gerontology Resources for APRN Preceptors and Students toolkit is to make geriatric and gerontological content accessible to those caring for older adults. It is designed with advanced practice registered nurse (APRN) students, preceptors, and educators in mind, and it provides a single portal to comprehensive resources. The toolkit is organized by primary care topics and includes links to websites and mobile device apps in the public domain.

This Acute Care Resource Guide is organized alphabetically by acute and emergent care topics and includes links to websites and videos available publicly.

During a meeting with members of the White House COVID-19 Response Team on October 17, 2022, leaders from healthcare associations across the country were asked to educate their members and stakeholders about the importance, effectiveness and accessibility of the COVID-19 bivalent booster and the therapeutics available to treat those diagnosed with COVID-19. AMDA-The Society for Post-Acute and Long-Term Care Medicine partnered to create this toolkit for clinicians working in post-acute and long-term care settings, treating the most vulnerable of our population.

The resources in this toolkit provide practical ideas for improving the care planning process. The first set of practice-oriented resources is for professionals who develop and write care and service plans while the "Policy and Practice" document provides recommendations geared toward policymakers and health plan officials.

About the Person-Centered Enrollment Strategies for Integrated Care Project: For individuals dually eligible for Medicare and Medicaid, misaligned incentives across these programs can result in higher costs and worse health outcomes. As a result, policymakers have been especially interested in developing integrated programs to better coordinate services.

The Center for Consumer Engagement in Health Innovation has created a toolkit to guide advocates as they assess and work with state’s Medicaid ACOs (Accountable Care Organizations). Each section contains a list of items to consider as Medicaid ACOs are designed and implemented. Creating this type of system takes time and is an evolving process. The most fundamental part of each component is to ensure that the consumer voice is present. Consumer advocates can play a role in the design, implementation and oversight of Medicaid ACOs.

The "Helping Indigenous communities stay connected in light of COVID-19" project worked with community partners to develop evidence-based and culturally safe and appropriate COVID-19 resources. An environmental scan of health promotion and informational materials was conducted early in the project to give a broader understanding of what health promotional materials were being developed for and with Indigenous communities internationally.
