This web page from the Alzheimer's Association aims to help family and friends of someone with memory changes to feel more confident in assessing the situation and taking action. The web page includes downloadable/printable note-taking guides for those who have noticed changes in themselves and those who have noticed changes in others. The guides provide example conversation prompts and resources for taking action. Both guides are available in English and Spanish. Links to relevant resources are included.
