The PREPARE for Your Care program, a public health awareness campaign led by a coalition of national and state health care leaders, aims to help people make plans in the event of illness. The goal of the campaign is to inspire people to make a hospital go bag, choose a medical decision maker, and talk to family about health care wishes. The program includes video stories and provides guidance for filling out an advance directive. COVID-19 resources are included. Content is available in English and Spanish.

This guide from The Conversation Project, an intitiative of the Institute for Healthcare Improvement, provides three important steps to take to be prepared in the time of COVID-19. Topics discussed include choosing a health care decision maker, talking about what matters to you, and thinking about what you would want if you become seriously ill with COVID-19.

MY STORY is a form that was developed by the Assisted Living Program of the Anne Arundel County Department of Aging and Disabilities to help families document personalized information about the needs and preferences of their loved one. Whether they live in an assisted living facility, nursing home, with a family caregiver, or in their own home, this form may follow a loved one to doctor’s appointments, hospital visits, and rehabilitation stays. An instructional video and guide for completing the form are available on the web page.
