MY STORY is a form that was developed by the Assisted Living Program of the Anne Arundel County Department of Aging and Disabilities to help families document personalized information about the needs and preferences of their loved one. Whether they live in an assisted living facility, nursing home, with a family caregiver, or in their own home, this form may follow a loved one to doctor’s appointments, hospital visits, and rehabilitation stays.
