The Survey of Organizational Attributes of Primary Care (SOAPC) was developed to assess health care professionals' perceptions of the organizational resources for change required in primary care settings and measures organizational attributes of practice teams as providers of quality of care.

The Survey of Organizational Attributes of Primary Care  is a 21-item tool with a 5-point scale that is used to assess health care professionals' perceptions of the organizational attributes required in primary care settings. The tool has 4 subscales: communication, decision making, stress/chaos, and history of change.
