Required Initiative Elements
Applications should not exceed 5000 words (equivalent to 10 pages in length). This does not include the executive summary, bio sketches, budget forms and letters of support.

The initiative:

Purpose & goals; why is this initiative unique and how will it contribute to advancing interprofessional education and collaborative practice in the school/university and nationally

Initiative description:

how students will develop the Interprofessional Education Collaborative Core Competencies for Collaborative Practice (IPEC competencies);

two-year timeline and milestones;
-
number of students involved and how you will ensure participation of at least 20% of the eligible students in the professional programs by the end of two years;

how IPC will be threaded throughout the curricula of the participating programs, not just one event or activity

Description of the opportunities of this initiative and how challenges in designing and implementing this IPE initiative in a community setting will be addressed

List the intended outcomes and how they will be measured

Describe the plan for sustainability and continued interprofessional and community collaboration after funding ends

Provide a statement of commitment to the National Center’s Nexus and Triple Aim of Alignment (See Questions & Answers)
Leadership & faculty:

Qualifications of identified principal investigator and key faculty, including alignment with initiative requirements, leadership experience and synopsis of past work in interprofessional education and collaborative practice.

How faculty are prepared now, and will be in the future, for IPE in community-based settings
-
How the initiative will build upon and partner with the state’s Future of Nursing State Action Coalition to advance best IPE practices.
Experience with IPE:

Description of existing interprofessional education and community collaborations (including extramural funding) and how participation in this initiative will accelerate IPE and community partnerships.

Current infrastructure in the participating schools/programs and the college/university to support IPE and community-campus partnerships and how that will contribute to the initiative’s success and sustainability.
Community Setting:

The nature of the community and population, and how the initiative meets the needs of the community it will serve.

Commitment from a community-based clinical partner to the initiative and potential future collaborations (also documented in a letter of support)
