Over the course of the two years of funding, the successful applicant schools must develop a core interprofessional clinical initiative in a community-based clinical setting that, by completion of the two-year project, will be rooted in the curriculum of the participating schools and the schools/universities, and sustained after the grant funding has ended.

The goal is a minimum of 20% student participation for all schools. Applicants must provide an explanation for the number of students they will recruit and how they will ensure that a minimum of 20% of the eligible students in the professional programs will participate in the program by the end of two years.

Applicant schools must have the administrative and financial capacity and experience to accept the award on behalf of the initiative and be able to comply with all financial and evaluation reporting requirements.

Initiatives must address the themes of interest:

Addressing factors that influence the social determinants of health, such as education, employment, transportation, environment, and community safety and security, as these factors impact the ability of individuals, families and communities to live the healthiest life possible (www.cultureofhealth.org).

Testing new innovative approaches to interprofessional education and collaborative practice in the community with both traditional and non-traditional professional partners.

In addition, proposals in a variety of community settings, providing comprehensive, coordinated, continuous, and expert care of older persons by improving the competence in geriatric care of a team of health professionals are encouraged.
Proposal Budget:
1. Applicants must prepare and submit budgets for both years of the project indicating how both the project funds and cash matching funds will be used.
2. Applicants must prepare and submit a budget justification for both year 1 and year 2.
3. Project funds ($50,000 grant and $50,000 cash matching funds for a maximum total of $100,000 over 2 years) can be used to support the initiative; support travel to and participation in the National Center’s mandatory three-day institute, New Models of Care Require New Models of Learning, on October 24-26, 2016; course planning, such as meeting/retreats; curriculum and faculty development; consultants; or funds to the community clinical site related to this initiative.
Matching Funds:
Successful applicants must demonstrate a 1:1 cash match.
- Matching funds must be documented with cash commitment letters from each organization providing matching funds.
- In-kind resources are also highly encouraged but cannot count toward the 1:1 match.
Letters of Institutional Support:
Applicants must demonstrate commitment from school administration (deans) and university administration (senior academic administrator of the higher education institution):
- Letter from the Dean of each participating professional school in which s/he assures: faculty and student participation in the initiative; facilitation of integration into the professional school’s curriculum by the end of the two-year grant period; and continuation of the initiative after the funding has ended;
- Letter from a senior academic administrator (e.g. President, Provost, and/or Vice President for health sciences) documenting their awareness and support of the initiative.
Community Partnership Support:
Applicants must provide a letter of support from the Executive Director or equivalent of the community-based clinical partner that assures they will actively work with the schools to develop a meaningful and sustainable interprofessional clinical initiative.
Biosketches:
Applicant must provide biosketches (maximum 3 pages) for the principal investigator and each key faculty/staff member.
