In this presentation (2:00:23 minutes), a brief ECHO orientation is given by Sandra DeBry, followed by Ellen Flaherty, who discusses long term services and supports related to the 4M's. By the end of the presentation, learners should be able to: (1) summarize the 4Ms of an age-friendly health system; (2) describe one example of how to bridge primary care and long term services and support using the annual wellness visit; and (3) identify one model focusing on "what matters most" in the community. Slides are available to download. Included in this presentation is a patient case given by Cherie Brunker. Part of the Age-Friendly ECHO series.
