This guide discusses key concepts related to providing culturally competent care to older adult patients. Chapters address issues and concenrs providing relevant details regarding belief,s traditions, and customs that would apply to clinical encounters with an older adult from 15 diverse ethnic backgrounds. The guide also includes topics relevant to intercultural care, including health literacy, approaches to clinican education, and the interface between spirituality and health decision-making.

In this chapter we describe the creation of a data repository, the National Center Data Repository (NCDR) and a national network of performance sites generating that data, the National Innovation Network (NIN) for the National Center for Interprofessional Practice and Education (hereafter the National Center). Interprofessional client-centered collaborative practice (ICCCP) is collective by nature, emerging as it does at the intersection of a wide variety of professional knowledge and scopes of practice. Many studies of ICCCP focus on the determinants or inputs of collaborative practice as well as on the results, outputs, or outcomes. This is echoed methodologically, as a preponderance of ICCCP teamwork studies primarily employ interview and survey data.

This book discusses the application of complex adaptive systems theory to the design and evaluation of patient-centered medical homes (PCMHs). The three defining goals of PCMHs are to spread patient-care roles among healthcare team members, focus on disease prevention and include the patient in the healthcare team. It explains why some PCMH pilots are highly successful while others do not show much benefit, covers specific sub-theories that allow for bracketing of different aspects of the clinic system and highlights strategies by which institutions can engage in this process.
