This webinar discusses how function and mobility impacts patients with a focus on older adults. This webinar goes over implementation principles, strategies to create urgency in care systems, and real world examples where these techniques have been applied.

Part I (Kellie Flood) Learning Objectives: 1) provide a brief overview of implementation principles for context; 2) provide examples of strategies for creating urgency in all disciplines within your care setting to own age-friendly care; and 3) summarize real-world examples of system-level care delivery redesign to hardwire safe mobility care processes.

Part II (Dr. Gibson): Learning Objectives (at 58 mins): 1) define how social determinants of health compound the risk for Alzheimer’s and other related dementias (ADRDs); 2) define the fourteen modifiable risk factors in early life, mid-life and late life that will delay and may prevent Alzheimer’s and other related dementias; 3) define the ten healthy habits to maintain and improve brain health; and 4) address the need for pubic health education of Alzheimer’s and other related dementias (ADRD).
