CATCH-ON Basics and 4Ms of an Age-Friendly Health System for Healthcare Professionals and Students

Evidence-based education that helps clinicians improve care.

Online modules: Available to all health professionals and community-based organizations, these modules developed by CATCH-ON educate about normal aging, managing multiple chronic conditions, dementia and working with interprofessional geriatric health care teams as well as the 4Ms of an Age-Friendly Health System (What Matters, Medication, Mentation, and Mobility). A version of these modules, aimed at older adults and their families, is available in English and Spanish.

CATCH-ON Learning Communities (LCs): These monthly one-hour videoconferences feature a clinician presenting a case relevant to that month’s geriatric primary care topic. A geriatric expert then provides a 20-minute lecture with specific behavioral recommendations that participants can use immediately, followed by the full group discussing application of the content. Behavioral Pain Management (BPM) training: This non-pharmacological intervention helps minimize opioid use and pain-related distress by teaching health professionals how to help older adults manage their own pain. Two levels of training include a one-hour training for all levels of clinic staff to help them talk with and refer older adults, and a three-hour interactive workshop for mental health professionals on implementing the evidence-based intervention program. Active Listening for Direct Care Workers: Direct care workers — many of whom are family members — spend more time with older adults in the home than any other care clinician. Teaching them the best ways to listen and act on What Matters to the older adults they care for will improve the care they provide along with their communication to families and health care clinicians. Health Education About LGBT Elders (HEALE) HEALE has been widely recognized that the older LGBTQ population is at risk for increased health disparities and suffers from poorer health outcomes relative to their heterosexual counterparts.  Providing culturally appropriate care for older LGBTQ people means implementing best practices for that population. Safe-Home Opioid Management Education (Safe-HOME): This literacy-tested training program provides education about the safe use of opioids and naloxone rescue medication. This training is currently available for care coordinators and direct care workers for use with older adults and their caregivers in the home. Mental Health Training

Essentials of Older Adult Mental Health training: A three-hour training for primary care clinics and community-based organizations on the essentials of older adult mental health is aimed at increasing knowledge and detection of late-life mental health issues.
