This learning module discusses healthcare teams. By the end of the presentation, learners should be able to: (1) discuss the reasons an interprofessional team can provide better care for an older adult with multiple chronic conditions (MCC); (2) identify the roles and responsibilities of possible common team members; and (3) describe how the individual older adult needs to determine who becomes a member of the team.
