Age-Friendly Care, PA is a collaboration between the Penn State Ross and Carol Nese College of Nursing and the Primary Health Network.

Age-Friendly Care, PA has developed online learning modules aimed to educate older adults and caregivers on the 4Ms of age-friendly care. The self-paced modules are intended to support the unique health goals and priorities that matter to you, your loved ones, and the people who care for you. You will be introduced to each of the 4Ms: What Matters, Medication, Mind, and Mobility. You will discover how each component of care relates to one another, learn tips on how to talk to your healthcare team about what's most important to you, and gain access to resources that support health and well-being as we age.
