Age-Friendly Care, PA is a collaborative between the Penn State Ross and Carol Nese College of Nursing and the Primary Health Network.

Age-Friendly Care, PA has developed online learning modules aimed to educate healthcare professionals on the 4Ms. The modules take an in-depth look at each M: What Matters, Medication, Mentation, and Mobility.  Important concepts, such as how to assess each element of age-friendly care, along with how to incorporate the individual needs of the older adult in their plan of care, will be addressed. Numerous resources are proviced to enable you to implement evidence-based geriatric best practices that correspond to the 4Ms. The online learning modules are self-paced and RNs can earn up to 5.5 contact hours for completing the modules.
