This educational module is designed byCollaborative Action Team Training for Community Health-Older Adult Network (CATCH-ON) as part of their 4Ms of Age-Friendly Health Care Program. By the end of this module, learners will understand: (1) why older adults are at greater risk for medication-related harms; (2) why older adults who take more than five medications per day need to be particularly careful; (3) medications that are highest risk for older adults; and (4) how to reduce medication-related harm.
