Presenter Bios:

Rosiland Harris, DNP, RN, APRN, RNC, ACNS-BC

Dr. Rosiland Harris is the Director of the Department of Professional Nursing Education, Practice, and Research from 2013 to present. She is the Principle Investigator and Project Manager of Project I-CARE. She oversees 18 educators and is primarily responsible for orientation, continuing education, an American Heart Community Training Center, nursing performance improvement, nursing research, and nursing policy development for a level one trauma center and teaching hospital with over 850 inpatient beds. Dr. Harris has also been the recipient for two previous HRSA grants to develop and expand RN Residency Programs at the institution. Dr. Harris is a published author with a research interest in medication error prevention.

Content Description:

The presentation will describe implementation, challenges, and lessons learned in a step wise unit by unit roll out of interprofessional clinical bedside rounds in a large teaching hospital with two schools of medicine. The hospital is a Level 1 Trauma Center with over 850 beds providing care to a medically underserved urban population. The objectives of the interprofessional rounding process is to improve the health and safety of at risk patients through comprehensive, team based, patient centered care planning focused on those with low socioeconomic status and limited health literacy. The importance of specific rounding processes such as pre-rounding and team debriefing will be discussed. The process of implementation and education of all health care professionals will be included. Oversight provided by grant facilitators and unit educators, development of web based rounding tools, and addition of a care plan template to the electronic medical record will be discussed as clinical enhancements. Interprofessional training including simulation, TeamSTEPPS principles, culture, and team based evaluation of rounds, and debriefing will also be reviewed. The innovative roles of a pain nurse specialist, patient education specialist, and a community health worker to meet unique patient needs will also be included. Finally, attendees will evaluate a videotaped I-CARE rounds using an observational checklist.

  1. Discuss implementation of interprofessional clinical bedside rounds including achieving buy in of all health professions, awareness training, roll out process, and support of rounds facilitators/educators.

  2. Identify challenges and actions taken to sustain the I-CARE rounding process as various units go live throughout the health system.

  3. Describe the interprofessional education/training implemented related to the I-CARE rounding process.

  4. Review the development and use of a comprehensive team based patient centered care plan.
