Person-Centered Dementia Care in a Hospital Setting

This learning module has three parts and discusses person-centered dementia care in a hospital setting. The first topic is person-center dementia care in the hospital setting. Learners should be able to: (1) list common reasons why persons living with Alzheimer's Disease and related Dementias (ADRD) are hospitalized; (2) describe what is meant by person-centered care and how it applies specifically to persons living with ADRD; and (3) discuss what are important factors to consider if a person who lives with ADRD has to be hospitalized.
The second topic is communication with people living with dementia in the hospital care setting. Learners should be able to: (1) discuss important things to consider when communicating with the hospitalized person living with ADRD; (2) use Mrs. Kemp as an example to illustrate issues related to care; and (3) develop a person-centered plan of care for a hospitalized person living with ADRD.
The last topic is maximizing function and safety in the hospital setting. Learners should be able to: (1) address function and safety considerations for hospitalized persons living with ADRD; (2) develop a person-centered plan of care for a hospitalized person living with ADRD; and (3) create a discharge plan that involves the individual the family, the caregivers, and the healthcare team.
