Included are downloadable PDF fact sheets / handouts created by REACH in both English and Spanish:
1. "Mental and Emotional Health" handout for patients and care partners on how to identify signs and symptoms of depression and provides possible treatment options.
2. "Responding to Behavioral Symptoms of Dementia" handout on how to respond in a helpful way to those you care for with behavioral symptoms of dementia.
3. "Delirium: When Confusion Can Be Dangerous" handout on how to differentiate between delirium and dementia.
