Case Study

Access more than 25 case studies involving real-life teams, patients, students, and families. These cases feature examples of successful IPP collaboration across a variety of settings with a variety of health and education professionals and community members. More than 35 different professions are represented among the interprofessional teams and cases thus providing a rich resource for preprofessional student learning as well as the professional development of workforce teams. Use the case studies to foster coordinated care, team communication and collaboration, role clarification, and reflection about team processes.

Each case study includes a brief narrative summary followed by a more detailed case description provided in a case rubric format.

  • How They Collaborated – a description of how the team is working together to develop an assessment and intervention plan
  • Outcome – of coordinated care achieved
  • Ongoing Collaboration - among team members 

Case Study Rubric (Detailed Summary)

  • Meet the Team – a listing of the health professionals that are collaborating on the case
  • History and Concerns – about the case shared by the team
  • Assessment Plan – roles and responsibilities of the team members for evaluation
  • Assessment Results – key diagnostic results shared by team members
  • IPP Treatment Plan – features team contributions to discuss and develop recommendations for a coordinated plan of care
  • Treatment Outcomes – results achieved
  • Team Follow-up – ongoing plans for team communication and follow-up
