As we live longer, we know we’ll need a health system that understands what makes our care unique as we age. The John A. Hartford Foundation, a national philanthropy committed to better care for older people, took one of the most important first steps toward that goal by coordinating an effort to develop “age-friendly health systems.” In this section of HealthinAging.org, you’ll learn more about making the most of age-friendly care for your own 5Ms (what matters most, the mind, medications, mobility, and multi-complexity), including ways to choose tests, treatments, and even care settings wisely. A list of age-friendly health systems is also provided along with other helpful resources.
