This learning module is about creating age-friendly health systems. By the end of this module, learners should be able to: (1) apply interprofessional and interconnected approaches of the Age-Friendly 4M Framework in a professional role; (2) describe a method to align care with an older adult's health outcome goals and preferences; (3) identify the need for de-prescribing and dose adjustments when using high-risk medications; (4) list benefits of identifying dementia in older adults, resources for assessment, and supportive interventions; (5) describe the relationship between normal aging and dementia; and (6) discuss how mobility affects the other three Ms of Age-friendly Health Systems. This learning module includes a pre-test, reading content, post-test, activity-specific evaluation, and activity evaluation. Registration is required by the creators to access the material but there is no charge. This module expires on April 19, 2023.
