By the end of this presentation, learners should be able to: (1) describe concepts and prevalence of age-related loneliness and social isolation; (2) discuss lessons learned from the COVID-19 pandemic; and (3) implement strategies for Age-Friendly Primary Care Health System Integration to include assessment of loneliness and social isolation as well as intervention strategies (case study).
