Aligning Resources to Provide for Aging Adults in the Community

Education-Community Partners:
University of Colorado College of Nursing and School of Pharmacy and Pharmaceutical Sciences. Community partner: Sheridan Health Clinic, a nurse-managed federally qualified health center (FQHC).


Focused Description:
The Colorado NexusIPE™ Team started with a vision to enhance care for a population of older adults served by the Sheridan Health Clinic, a nurse-managed federally qualified health center (FQHC), especially focused on health literacy and patient safety while expanding practical and innovative IPE for students in a clinical environment.  The Nexus team incorporates pharmacy and undergraduate and advance practice nursing students to provide home visits to older adults who are served in the FQHC. The initiative met a gap in patient care needs by providing outreach to the population of older adults. Students add value through identification of near misses and enhance the quality of care for the population. The NexusIPE™ team created a system to successfully schedule follow-up appointments with referrals to primary care, behavioral health, psychiatric and dental services scheduled as a direct result of home visit. In the process of filling a gap in needed services, all NexusIPE™ partners were able to realize outcomes.
