Focused Description: In Arizona, as elsewhere, the steep rise in opioid drug use has put a strain on community services. Often, individuals in substance abuse treatment face complex challenges regarding physical health, mental and behavioral health, and social barriers such as homelessness that make recovery difficult and relapse common.

The Arizona NexusIPE™ initiative accelerated the expansion of a successful tri-university, student-led, faculty-mentored, and community-based Student Health Outreach for Wellness (SHOW) model of learning and care to a high-risk rapidly growing population of individuals with substance abuse disorders. Three student teams (health management, health promotion, and leadership) formed to meet goals by working closely with the staff and Executive Director of Crossroads to ensure the community partner’s needs were being met. Prior to this program, Crossroads residents did not receive integrated direct care or health promotion services. Learn how the community partner’s needs were met while providing previously unavailable services to Crossroad’s residents.
