Nexus Success in Nebraska: Invested Leaders and True Partnerships

Focused Description:
Education and community partners aligned with a common commitment to improve the health of older adults by focusing on enabling their safe transitions from hospital to skilled nursing facilities (SNF). The program brings together SNF staff, graduate nursing and pharmacy students, and their preceptors.  The interprofessional team assesses SNF residents who are on multiple medications for multiple chronic diseases (polypharmacy) to address their needs related to appropriate medications with the goal of to preventing adverse events and readmissions after hospitalizations. In the first cohort of patients, the interprofessional team was able to successfully discontinue inappropriate medications on the Beers Criteria for Potentially Inappropriate Medication Use in Older Adults, adjust dosing, and appropriately add needed medications correlated with lab results. No adverse events occurred for residents seen by the interprofessional teams in the first 30 days of the initiative.

The presenters will provide insight into how this partnership was different than past partnerships, what had to change in order for them to be successful, and how their focus on the needs of the older adults in transition contributed to benefits for all; including the patients, the education partner and their students, and community partners and their staff.  The presenters will also highlight the NexusIPE™ tool, Creating and Communicating your Compelling Vision tool and how this tool contributed to their success.
