The Institute for Continuing Education of the 1199SEIU League Training and Upgrading Fund in collaboration with the 1199SEIU United Healthcare Workers East Professional and Technical Department; National Center for Interprofessional Practice and Education; Columbia University Medical Center, College of Physicians and Surgeons Program in Narrative Medicine; National Association of Social Workers-New York State present this seven-hour interprofessional program. 

The program will focus on the changing national healthcare environment and the importance of interprofessional, collaborative, team-based practice in improving patient experience and health. The program has been accredited for a number of healthcare practitioners, including physicians, nurses, pharmacists, physician assistants, and others.

Overview & Purpose: Developed within the context of the dynamic and evolving Medicaid delivery system reform landscape, including the New York State Delivery System Reform Incentive Payment (DSRIP) program, which calls for enhancing the experience and outcome of the patient and tying payment to higher value care, changing the way care is delivered through greater teamwork and integration, more coordination across settings and, in general, greater attention to population health improvement.

Key Areas of Discussion: 

• Why is the health care system changing?

• What are the federal and statewide policies that affect this work, and what are the rewards and implications of achieving the established intended outcomes?

• Why are interprofessional teams important? And how might those teams be different from the types of teamwork health care workers are currently engaged in?

• How can teamwork improve outcomes and reduce hospitalizations for specific populations, such as individuals with diabetes, asthma, obesity, hypertension, or individuals who are impacted by mental and behavioral health issues?

For seven (7) contact hours: This activity is planned and implemented by National Center for Interprofessional Practice and Education, the 1199SEIU League Training and Upgrading Fund and 1199SEIU United Healthcare Workers East Professional and Technical Education Committees. The National Center for Interprofessional Practice and Education is accredited by the Accreditation Council for Continuing Medical Education (ACCME), the Accreditation Council for Pharmacy Education (ACPE), and the American Nurses Credentialing Center (ANCC) to provide continuing education for the healthcare team.
