Assessing the Practice in Partnership with Patients & Families: Opportunities to Improve Patient and Family-Centered Care

Redesigning your clinical practice can be overwhelming. Where do you start? How do we prioritize our quality improvement efforts? Patients and family caregivers can provide crucial insight that will help you make these decisions. Patients usually know what is working for other patients in the practice and what changes could make a big difference in making the practice more patient- and family-centered. Sometimes the greatest impacts on improving care through the eyes of the patient only requires modest effort by the practice.
