Form Nexus Innovation Challenge Application

You are invited to apply to the Nexus Innovation Challenge: Design Thinking to Improve Interprofessional Environments.

Teams of 5-8 members from one or more institutions working together are encouraged to apply.  The application centers around your team identifying a challenge idea within your system (network, clinical unit, partnership) and develop a challenge statement that will be the focus of your design work and action plan.  If you have fewer than 5 members and would like to participate, we encourage you to complete the application process.  Our coaches will follow up with you to explore how to support you in ensuring your team has representation from stakeholder groups important to achieving your goals.

Challenge Idea

Briefly describe the problem or opportunity your team would like to address during the Nexus Innovation Challenge. The scope of the problem should be something that can be implemented within about a six-month timeframe. Elements of a successful challenge idea should include:

  1. Interprofessional collaboration among health professionals and/or other key stakeholders or sectors;
  2. Specific goal to address diversity, disparities, equity and racism;
  3. Intentional engagement of patients, families, caregivers, communities, populations; and
  4. Support of leaders within the organization committed to working toward galvanizing systemic change.
