Xavier University Health Conference: Tomorrow's Coordinated Health Care System - Today

"Where the puck is going - reformed payment systems, inter-professional population health"

 

Conference Goal: Update national healthcare on efforts to transform healthcare delivery to a culture of inter-professional collaboration that meets the Triple Aim goals to enhance the patient's experience and improve health outcomes at lower costs. Best practices will be shared in breakout sessions.

Conference Objectives:

  1. Discuss innovative inter-professional collaborative approaches for population health care delivery to diverse populations.

  2. Describe payment reform and models for delivery of coordinated care to diverse populations.

  3. Explore current and future CMS considerations for population health delivery.

  4. Discuss integrated models of care using a care management team approach.

Free. Conference costs paid by funds from the U.S. Department of Health and Human Services, Health Resources and Services Administration (HRSA) under cooperative agreement # UD7HP28544-01-00 and title Nurse Education, Practice, Quality, and Retention (NREQR) Inter-professional Collaborative Practice $1,461,223.00: Budget period 07/01/2015 through 06/30/2018
