Partnerships between Clinicians and Community-Based Organizations to Improve Patient Outcomes
Join National Health Officer from YMCA of the USA (Y-USA), Matt Longjohn, MD, MPH together with Omar Hasan, MBBS, MPH, MS, FACP from the American Medical Association (AMA) to learn about the Center for Medicare and Medicaid Innovation (CMMI) demonstration project to deliver the YMCA’s Diabetes Prevention Program to nearly 8,000 Medicare recipients and the partnership between the Y and AMA to support these efforts.
This three-year project led to the March 23 announcement by U.S. Department of Health and Human Services (HHS) that the successful demonstration project has been shown to produce significant cost savings for Medicare participants, marking a critical step for HHS to eventually expand the Diabetes Prevention Program under Medicare for those with prediabetes.
