The Survey of Organizational Attributes of Primary Care (SOAPC) was developed to assess health care professionals' perceptions of the organizational resources for change required in primary care settings and measures organizational attributes of practice teams as providers of quality of care.

This tool can be used to measure proximal work group climate and team members' perceptions of climatic dimensions in organizational climate surveys, team building and development, selection of new members into groups, and group development over time.

The Team Decision Making Questionnaire is  a 19-item tool with a 7-point scale that is used to assess the quality of  interprofessional teams. The tool has 4 subscales: decision‐making, team support, learning, and developing quality services. 

PINCOM-Q: Perception of Interprofessional Collaboration Model Questionnaire
