This learning module is about creating age-friendly health systems.

HSC Health: High-Risk Medications

This learning module addresses the characteristics of medications that are high-risk and how to mitigate those risks for older adult patients. By the end of this modules, learners should be able to: (1) review the characteristics of a 'high-risk' medication; (2) list medication groups considered high-risk for the older adult; (3) apply age-friendly framework to patient-centered prescribing and deprescribing; and (4) review the Merit-based Incentive Payment System (MIPS) measure for high-risk medication in the elderly.

Community and Clinical Issues

Answering the vexing questions about organizing IPE in higher education

The road to interprofessional education (IPE) and interprofessional collaborative practice (IPCP) in North America is described as a “long and winding” one. Most in our field are unaware of how long our history truly is, and why commitment to teams, IPE, and IPCP comes and goes over time, creating short institutional memories. Valuing health care teamwork dates to the 1910s followed by a significant surge of interest after World War II.

Age-Friendly Care, PA is a collaboration between the Penn State Ross and Carol Nese College of Nursing and the Primary Health Network. 

Age-Friendly Care, PA has developed a website that focuses on the 4Ms of age-friendly care. The website provides an overview of the 4Ms, offers a comprehensive list of tips and tools organized by the 4Ms, shares the My 4Ms Worksheet and instructional video, and serves as the portal to our online learning modules.
